Child registration forms (under 18) These forms are also helpful for young adults living at home, especially if there are developmental issues still of concern. Please fill out the first two forms - Registration and Child Clinical Questionnaire - read the HIPPA information about your federally mandated privacy rights - sign the HIPPA form that acknowledges that you received the HIPPA information - and bring all the forms with you to your first appointment.
