Download the Adult or Child forms below, fill them out, and bring them with you to your first appointment. Read the HIPPA policies and bring the signed Acknowledgement with you as well. Feel free to expand on any answers. If a question doesn't apply, write N/A or leave it blank.
If your insurance requires pre-authorization for this visit, please make sure you contact your insurance carrier or your primary care physician.
